Federal DOT Change to Chain-of Custody Form

On July 1, 2018 the new Federal DOT chain-of-custody form (CCF) will be required at all drug screen collection sites. If odd CCF's are used after June 30, 2018 a memorandum from the laboratory must be completed by the collector and this may result in delay of reporting. If Sanford OccMed is your MRO there is nothing you need to do.

What do I need to do if my MRO is someone other than Sanford OccMed?

1. Contact the administrator of your drug testing program prior to June 30, 2018.
2. Verify that you have copies of the new DOT CCF.
3. Throw away any old DOT forms.
4. Bring the new copies into Sanford OccMed at the time of service.
