Federal DOT Change: Chain of Custody Forms

On July 1, 2018, the new Federal Department of Transportation (DOT) Chain of Custody Form (CCF) was required at all drug screen collection sites. If old CCF’s are used after June 30, 2018, a memorandum from the laboratory must be completed by the collector, and this may result in a delay of reporting. If Sanford OccMed is your medical review officer (MRO), there is nothing more you need to do.

What do I need to do if my MRO is someone other than Sanford OccMed?

1. Contact the administrator of your drug testing program.
2. Verify that you have copies of the new DOT CCF.
3. Throw away any old DOT CCF forms.
4. Bring the new copies into Sanford OccMed at the time of service.
